Students, you should receive copies of these forms in an informational packet that will arrive soon. You should fill out the medical form, letter of understanding, field trip permission form, and band forms (optional); and give them to your Boys State Counselor when you arrive at Boys State. If you wish to purchase a yearbook, bring the completed order form along with payment to Boys State and give it to your counselor when you arrive.
Students, if you are interested in running for the House of Representatives or the Senate, you should prepare a bill to sponsor and bring it with you to Boys State. Please go to this page for more information.

There are two separate yearbook forms -- one for sponsors, and one for students. Students: Many sponsors purchase yearbooks for the students they sponsor, so check with your sponsor.
If you play a musical instrument and would like to play in the Boys State Band please fill out the enclosed form and return it the address on the form that you can download below.
